So a big issue with the current weapon and armor set up is at the rate they are going there would be too many in order to support the level cap (especially if the level cap is increased) ...
My idea is a final Weapon/Armor set that grows with the player...
Currently the strongest weapon and armor are the Demon Forged Set (DFS), 650 effectiveness: doubled as Soulcharged.
At the current level cap (250) a player would unlock the ability to use this new set at level 200 (same as DFS, but the stats on it would lower initially)
As the player gains levels toward the cap 250 (and beyond) the stats on the weapon change...
Name for these items?
Let's go with Affinity Blade (dual wield) and Affinity Guard for now...
So initially at level 200 (the first level they can be used) the effectiveness would be around 600 (double if Soulcharged) or...
Effectiveness = Current Player Level * 3
so for each level these item gain 3 effectiveness so at level 250 they would have an effectiveness of 750.

This opens some other avenues to consider:
1. How do you get these items? Crafting or other?
2. What will the cost be?
IF not just added to the list of craftable items, there could be a questline where you gather materials to take to an NPC who combines them into the Affinity gear. ~credit to Lillith for this idea
Another idea to consider is varying stages, like when you first get the items they are not 3X Effective but start as only 1X and must be slowly upgraded until it is. (1X, 2X, 3X, 4X, etc) With the lower stages having lower requirements.
Affinity gear 1X (Eff = lvl) : Req lvl 1
Affinity gear 2X (Eff = lvl * 2): Req lvl 75
Affinity gear 3X (Eff = lvl * 3): Req lvl 200
Affinity gear 4X (Eff = lvl * 4): Req lvl ??? (raise lvl cap)
So these items would not pay off in effectiveness (as compared to other items) until near lvl cap.

This idea just keeps getting better and better, definitely in support WW! I think that although there doesn't seem to be any need for it now, once (more like when) the level cap is raised we will need equipment to match.
Another idea, is maybe that you still craft the equipment, but as you complete the questline the equipment gathers more and more power, until finally it is
